Practice Information and Policies
We ask that each new client complete the below documentation before beginning services. Each new client packet contains one of each of the below documents, with the exception of the release of information, which requires two to be completed before services can be rendered.
This is simply our warm welcome to any potential new clients. Some frequently asked questions are also addressed for easy access to information.
Informed consent is acknowledging that you would like to voluntarily receive the services that we can provide to you. Prior to beginning treatment, informed consent must be signed.
We understand that this is asking for a lot of information. Please understand that we ask these questions only to be able to better assist you in your mental health journey. Please complete as best and as fully as you are able.
The Health Insurance Portability and Accountability Act (HIPAA) of 1996 establishes federal standards protecting sensitive health information from disclosure without patient's consent. This simply means that what is said in our practice will stay in our practice unless we are concerned for your safety or the safety of others.
Our business policies cover confidentiality, sessions, cancellations and no shows, payment, and phone consultations. Please read each section carefully prior to signing
We understand that this form may be challenging to complete, so please let us help! We will complete the bottom two boxes. Please just complete the top two as best you are able.
Releases of information allow us to send and receive information with your other providers. We will need at least two, one for your family doctor (primary care physician) and one for your primary insurance company.
